Form 3: Membership Application

Use Form 3 to apply for membership in the Sons of Union Veterans of the Civil War or to submit a supplemental ancestor application.

The application includes sections for membership selection, applicant information, fees and dues, Civil War ancestor service, lineage documentation, and the Oath of Obligation. Instructions for completing and submitting the application are included with the form.

Application Options

• Initial Membership Application: For individuals applying to join the Sons of Union Veterans of the Civil War.
• Supplemental Ancestor Application: For current members who wish to register an additional qualifying Civil War ancestor.

• Associate
• Junior Member
• Junior Associate

Before Submitting the Application

• Review the instructions included on pages 5 and 6 of the form.
• Complete the sections required for the selected membership type.
• Sign the application in the presence of a witness.
• Applicants under 18 must have a parent or legal guardian serve as the witness.
• Hereditary applicants must include documentation supporting the qualifying ancestor’s Civil War service and the applicant’s lineage.
• Confirm the applicable application fee and dues before submitting payment.

Applicants are encouraged to apply through a local Camp or state Department. Individuals who do not know which Camp or Department to contact should email the Junior Vice Commander-in-Chief at [email protected] for submission instructions.
